AI visibility audit worksheet: record the answer, source, and next action

By the MentionedOn team

How to check business discovery, citations, and agent tasks on individual platforms.

A repeatable worksheet for comparing brand mentions, recommendations, citations, and agent task outcomes without confusing them with search rankings.

Open the public Google Docs worksheet or download the plain-text worksheet. Copy the blank fields into your own private working document.

Start with one buyer decision

Use this worksheet when a customer might ask an assistant to recommend a business or compare a product. Choose one service, one audience, and one location. Write the question before opening the assistant so you do not keep changing it until your brand appears.

Keep a private working copy. Record only public business information in any version you publish. This is a blank method, not a report of measured rankings or customer results.

  • Business and website: ______
  • Buyer, service, location, and language: ______
  • Exact unbranded buyer question: ______
  • Separate branded fact-check question: ______

Capture one row per completed answer

Record the platform, visible model or mode, time, and whether search was used. Keep an answer capture and the URLs actually cited. A brand name in a paragraph, a recommendation, a source citation, and a successfully completed booking are different observations.

Use the same question in a fresh conversation for a repeat check. If memory or connected accounts influence a session, label it personalized. If you cannot access a platform, mark it unavailable rather than recording no mention.

  • Platform / mode / capture time: ______
  • Search used: yes / no / unknown: ______
  • Memory, location, or connected-account context: ______
  • Answer or saved capture: ______
  • Brand mentioned / recommended / cited: ______
  • Exact cited URLs and competing businesses: ______
  • Incorrect fact or missing information: ______
  • Task attempted / completed / not tested: ______

Choose the smallest justified improvement

Open the cited sources. Identify the specific missing fact: service area, price basis, eligibility, evidence, availability, or how to contact the business. Fix that fact on the most relevant existing page and link it from the pages customers already use.

Do not create multiple near-identical articles for one question. Do not describe a self-authored comparison as an independent award. A repeatable method and clear limitations are more useful than an unsupported number-one claim.

  • Observed gap and supporting capture: ______
  • Page to improve and exact proposed change: ______
  • Owner and next review: ______
  • Success measure: accurate answer / citation / referral / qualified action: ______

Compare like with like

Repeat the same question set with the same location and mode. Report mentions as a count out of completed comparable checks; list failures and unavailable checks separately. One appearance is an observation, not a stable rank. Keep referral visits and conversions separate from assistant answer captures.

MentionedOn can help organize AI visibility work. Use only the monitoring surfaces supported by your current account. Treat Instinct, Muse, and Manus checks in this worksheet as manual research unless a specific integration has been separately verified.
